Left: End-stage heart failure affects an increasing part of our aging population. However, the invasive nature of existing, full support left ventricular assist devices is associated with a high rate of complications. Partial support allows miniaturization of devices and less invasive implantation procedures. We explored the use of a newly developed partial assist device for patients with chronic heart failure. This picture illustrates the anatomic position of the first in men implantation of the Circulite Synergy Micro-pump. The inflow cannula, implanted through a small antero-lateral thoracotomy, derives blood from the left atrium. The outflow graft is sutured to the subclavian artery. The pump itself is located in an easy accessible, subcutaneous pocket. Antero-posterior and lateral chest X-ray views with the Synergy pump in the right subclavian region and the metal tip of the inflow cannula in the left atrium are also shown.

Right: Handmade Dacron conduits that incorporate a trileaflet PTFE valve have been used for pulmonary reconstruction since 1997.
A 0.1mm PTFE membrane is cut in a rectangular shape, folded, and separated by suture lines to create three pockets. It is then rolled and both sides are sutured together to create a syringe-shaped structure with three separate pockets on the inside. The valve is pushed inside and fixed onto the Dacron conduit by suturing the top and bottom ends of the PTFE valve. After fixing the valve, the outer layer of the PTFE valve (at midpoint of the sinus portion of the valve) is sutured to the conduit to avoid free-floating. Commissural sutures (1mm distant from the commissure) are finally placed to improve valve coaptation.
These conduits showed acceptable functional durability, and maintained valve motion up to 10 years in all patients in this series.
